Here’s a simple version for a 3rd grade reading level:

We help keep people healthy with:

• Shots for kids and adults

• Tests and help for STDs and HIV

• Teaching about health

• Stopping sickness like TB

• Keeping food and water safe

• Getting ready for emergencies

We give shots and STD tests to:

• People with no insurance

• People with Medicaid

• Anyone who needs an STD test

For shots: Bring your Medicaid card and your shot record.

For STD testing: You don’t need to bring anything.

New patients: Come to the office to fill out papers. We’ll call you to set up your visit!
